Terms and Conditions of Medical Engagement

1. Clinical Services & Medical Disclaimer

Neuregen Scottsdale LLC provides interventional neurology, cellular regenerative medicine, and integrative psychiatric consultations under the direct clinical supervision of Dr. David George. All information presented on this application (including the Interactive Neuro-Path Finder, biological diagrams, and automated clinical representative chat responses) is strictly for educational, informational, and preliminary screening purposes. It does NOT constitute formal medical diagnosis, diagnostic prognostications, or clinical treatment advice. A formal physician-patient relationship is only established upon the execution of a signed Written Consent to Treatment form and an in-person physical clinical diagnostic examination.

2. Patient Registration, Disclosures & Authenticity

If you utilize our online intake portal or diagnostic assessment tools, you represent and warrant that all biological data, symptom profiles, contact details, and psychiatric history provided are fully accurate, complete, and current. You agree that Neuregen and Dr. David George may rely on the absolute accuracy of these disclosures for safe pre-screening of treatment eligibility. Providing intentionally misleading clinical data may result in immediate termination of therapeutic services.

3. Professional Scheduling, Deposits & Cancellation Policy

Interventional neurological procedures (including Dual Stellate Ganglion Blocks, sub-dissociative ketamine infusions, and cell-signaling exosome protocols) require dedicated sterile preparation, precision ultrasound equipment calibration, and specialized clinical staff scheduling. Accordingly, Neuregen enforces a strict 48-hour cancellation and rescheduling policy. Appointments cancelled or rescheduled with less than 48 hours notice may be subject to a late cancellation fee. No-shows for scheduled surgical/interventional appointments will forfeit any pre-paid treatment deposits.

4. Out-of-Network Operations & Financial Policy

Neuregen Scottsdale LLC operates strictly as an out-of-network private specialty facility. We do not participate directly in, or accept assignment from, commercial health insurance providers or government programs (such as Medicare or Medicaid). The patient is fully responsible for all clinical fees at the time of service. Neuregen will provide an itemized medical Superbill featuring standard ICD-10 and CPT codes upon request, which you can submit to your insurance company for potential direct-to-patient out-of-network reimbursement. Neuregen does not guarantee reimbursement or provide legal representations regarding insurance coverage rules.

5. Digital Telehealth and Online Pre-screening

Our online portal utilizes advanced interactive logic to assist in clinical pre-screening. By engaging with these digital pre-screening tools, you acknowledge that remote assessments cannot replace in-person diagnostic imaging, neurological physical testing, or psychiatric clinical interviews. Telehealth consultations conducted by our clinical team are subject to the regulations and medical licensing laws of the state of Arizona and/or the patient's local state where legally permitted.

6. Limitation of Liability & Indemnification

To the maximum extent permitted by applicable law, Neuregen Scottsdale LLC, Dr. David George, and their clinical officers, employees, or medical affiliates shall not be liable for any direct, indirect, incidental, special, or consequential damages arising from the educational use of this application or reliance upon pre-screening outputs. You agree to defend, indemnify, and hold harmless Neuregen and Dr. David George from any legal claims or expenses resulting from your violation of these terms or misuse of diagnostic information.

7. Governing Law and Jurisdiction

These Terms and Conditions shall be governed by, and construed in accordance with, the laws of the State of Arizona, without giving effect to any principles of conflicts of law. Any legal dispute, arbitration, or litigation arising out of or in connection with Neuregen’s clinical operations or website use shall be resolved exclusively within the state or federal courts located in Maricopa County, Arizona.
